Retin-a Cream (Tretinoin)
Retin-a Cream (Tretinoin) is a derivative of vitamin A. It is used on the skin (topically) in the treatment of mild to moderate acne and on skin that has been damaged by excessive exposure to the sun. Tretinoin irritates the skin and causes the cells of the skin to grow (divide) and die more rapidly, increasing the turnover of cells. The number of layers of cells in the skin actually is reduced. In patients with acne, new cells replace the cells of existing pimples, and the rapid turnover of cells prevents new pimples from forming. By a similar mechanism, tretinoin can reduce some wrinkles, areas of darkened skin (hyperpigmentation), and rough areas of skin, all of which occur in sun-damaged skin. In patients with sun-damaged skin, improvements in the skin usually are seen within the first 3 to 4 weeks of treatment. Brown spots begin to fade after six to eight weeks. Wrinkles decrease or disappear after three to six months. Following application to the skin, a minimal amount of drug is absorbed into the body.
BEFORE YOU USE THE MEDICATION
You should not use Retin-A if you are allergic to tretinoin. It is not known whether Retin-A will harm an unborn baby. Tell your doctor if you are pregnant or plan to become pregnant while using this medication. It is not known whether tretinoin topical passes into breast milk or if it could harm a nursing baby. Tell your doctor if you are breast-feeding a baby.
HOW TO USE
Retin-a Cream (Tretinoin) works by unblocking the clogged follicles that cause cystic acne. In treatment, they’re usually used together with antibiotics. As the tretinoin opens up clogged follicles, the antibiotics enter and get rid of the bacteria that cause acne breakouts. Tretinoin is usually applied in a thin layer on the skin that’s affected by acne once daily at bedtime for as long as the acne breakout lasts. Before you use tretinoin, wash your face with a mild soap and gently pat it dry. Wait 20 to 30 minutes before applying the medication. When using tretinoin, be careful not to get it into your: eyes, ears, nostrils and mouth. You can use cosmetics, but you should always wash your face before applying tretinoin.
A brief sensation of warmth or stinging may occur immediately after applying the medication. Skin redness, dryness, itching, scaling, mild burning, or worsening of acne may occur during the first 2-4 weeks of using the medication. These effects usually decrease with continued use. A daytime moisturizer may be helpful for excessive dry skin (see Notes). If any of these effects persist or worsen, notify your doctor or pharmacist promptly. Your doctor may want you to decrease how often you use tretinoin, change the strength or type, or have you stop using it. Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects. Tell your doctor immediately if you experience: blistering, crusting, severe burning/swelling of the skin, eye redness and watering (conjunctivitis), eyelid swelling, skin discoloration. A very serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction may include: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.
Which drugs or supplements interact with Retin-a Cream (Tretinoin)?
Combining other topical acne medications (for example, salicylic acid) with tretinoin may lead to excessive skin irritation. Use of abrasive soaps or cleansers, astringents, skin waxes and other products that irritate the skin may add to tretinoin-induced skin irritation. Medications [for example, tetracycline (Achromycin)] that cause sun sensitivity should not be combined with tretinoin because of additive sun sensitivity.
What should I avoid when using Retin-a Cream?
Avoid exposure to sunlight or tanning beds. Retin-A can make you sunburn more easily. Wear protective clothing and use sunscreen (SPF 30 or higher) when you are outdoors, even on a cloudy day. Avoid getting this medication in your eyes, nose, mouth, or in the creases of your nose. Avoid using skin products that can cause irritation, such as harsh soaps, shampoos, or skin cleansers, hair coloring or permanent chemicals, hair removers or waxes, or skin products with alcohol, spices, astringents, or lime. Avoid using other medications on the areas you treat with Retin-A unless your doctor tells you to.
